I am writing to formally request a complete copy of my child’s educational records pursuant to the Family Educational Rights and Privacy Act (FERPA) and the Individuals with Disabilities Education Act (IDEA), if applicable.
Please include all records maintained by the school and district, including but not limited to:
- All evaluations, assessments, and testing protocols
- Individualized Education Programs (IEPs), 504 Plans, and related documents
- Progress reports, report cards, and data tracking
- Service logs and related service provider notes
- Emails, correspondence, and internal communications referencing my child
- Behavioral records, incident reports, and disciplinary records
- Meeting notices, prior written notices (PWN), and meeting notes
- Any documents considered by the PPT/IEP team
- Any other records maintained in any format (electronic or paper)
If any portion of these records is withheld, please provide the legal basis for withholding.
I request that these records be provided as soon as possible, and no later than the 45-day timeline required under FERPA. If there is an upcoming PPT/IEP meeting, I request that records be provided in advance of that meeting so I may meaningfully participate.
